There are situations where two (or more) emails are associated with one contact. This happens when someone edits a contact record by changing the email address, instead of adding a new one. In your example, "dchamberlain@" was the original email name. It was later changed to "lchamberlain@". Finally, the contact record was later inactivated for an invalid email address.
Here's what I suggest you do. Search for "lchamberlain@..." You will get no results. But then select Search Options>Show Inactive Contacts. The name "Lee Chamberlain" should appear. Select that contact. Then select "Contact is still at company". Then select "Update business card info". Change the first name to David, edit the job title if needed, and then change the email address back to "dchamberlain@..." Then select Save.

You are totally right, there is Davids profile in history of changes, seems like I can just use the "revert" to make the contact with right name and even right direct dial back, and then just refresh the Title.
